Why You Shouldn't Be Wondering What Your Cortisol Level Is

Here's what cortisol actually does, when testing is medically appropriate, and why chasing a single cortisol number is rarely helpful.

Somewhere in the last few years, cortisol went from an obscure hormone most people couldn't have named to the villain of every wellness feed. "Cortisol face." "Cortisol belly." "This raises your cortisol, avoid it." I want to walk through why almost none of that reflects how cortisol actually works, and why asking "what's my cortisol level right now" is, for the overwhelming majority of healthy women, the wrong question entirely.

Cortisol is supposed to go up and down, constantly, all day

Cortisol isn't a number that should sit still. In healthy adults, it follows a robust and entirely predictable daily rhythm, starting to rise around 3 to 4 a.m., peaking around the time you wake up—typically between 10 and 20 micrograms per deciliter—and then declining steadily throughout the day to its lowest point near midnight. There's even a specific, well-documented spike called the cortisol awakening response, a rapid 38 to 75% jump in the first 30 to 45 minutes after you wake up. That's not dysfunctional. That's your body appropriately mobilizing energy and alertness to meet the day ahead. A cortisol level measured at 8 a.m. and one measured at 8 p.m. are supposed to look completely different, which is exactly why a single cortisol number, pulled out of context, tells you almost nothing useful.

This is also why at-home cortisol tests marketed as a single snapshot of your stress level are fundamentally misleading. Cortisol also varies by age, sex, and even season; one large dataset of nearly 19,000 people found levels run higher in autumn/winter and lower in spring/summer. A number without its time stamp, and without a clear clinical question behind it, isn't data. It's just noise dressed up as insight.

What cortisol testing is actually for

Clinically, we test cortisol for two specific reasons: suspected adrenal insufficiency (meaning your body isn't making enough) and Cushing Syndrome (meaning it's making too much), both due to genuine underlying conditions. These are real, testable, treatable diagnoses, and they require an actual clinical suspicion and a proper protocol—often multiple measurements at specific times of day, not a single wellness-app number. Cortisol testing was never designed to answer "am I too stressed," and using it that way stretches a diagnostic tool into a role it was never built for.

The HIIT example is where this really falls apart

Here's where social media has done real damage. Women have been told, repeatedly and confidently, to avoid high-intensity interval training because it "raises cortisol," as if that were self-evidently bad. What consistently gets left out of that warning is the concept of hormesis, the idea that a stressor, applied appropriately, triggers an adaptive response that leaves you stronger than before. Exercise is a textbook example. Yes, a single hard workout produces an acute cortisol spike. That's your body mobilizing energy for the effort, exactly the way it's supposed to. But research on consistent training tells a completely different story about what happens over time: Regular training, including HIIT, has been shown to lower resting cortisol levels and improve the balance between your stress hormones and your anabolic hormones like testosterone and DHEA. The acute spike is real. It's also temporary, and it's the mechanism that produces the long-term benefit, not evidence of harm.

This is the piece influencers consistently miss, not out of malice, usually just out of a lack of training in how endocrine systems actually behave over time. An acute rise in a stress hormone is not automatically dangerous. Context and time course are everything.

What actually warrants a Cushing workup

Since I keep saying "real underlying condition," I want to be specific about what that actually looks like. It's a far higher bar than "I feel stressed" or "my ring felt tight this week." True Cushing syndrome testing is reserved for people with multiple, progressive physical features together, things like facial redness/plethora, a rounded face, a fat pad at the base of the neck, wide purple stretch marks, muscle weakness in the hips and shoulders, easy bruising, and thinning skin. It's also indicated for unexplained osteoporosis or fragility fractures in someone under 50, resistant high blood pressure in a young adult, an adrenal mass found incidentally on imaging, or, in children, a growth pattern that's slowing while weight gain accelerates. When testing is genuinely indicated, it's not a single blood draw. It typically involves late-night salivary cortisol, a 24-hour urine collection, or an overnight dexamethasone suppression test, with two to three separate measurements—because even in real disease, cortisol naturally varies by as much as 50% from one sample to the next.

It's also worth knowing that several common conditions, including major depression, alcohol use disorder, obesity, PCOS, and poorly controlled diabetes, can mildly activate this same system and mimic some biochemical features of Cushing without it actually being Cushing. This is exactly why routine cortisol screening isn't recommended for most people, including those with diabetes: the false-positive rate is simply too high to make it a useful blanket test. None of this is a diagnosis you back into from a wellness app or a single number. It requires a specific, progressive clinical picture your doctor is trained to recognize.

So, what should you actually pay attention to?

If you have real, persistent symptoms, unexplained weight changes, significant fatigue, or other signs consistent with an actual adrenal condition, that's a conversation for your doctor, with an appropriately timed, clinically indicated test. Outside of that, chasing a cortisol number is chasing a hormone that was designed to fluctuate, reacting normally to your sleep, your meals, your workouts, and the time of day. You don't need to know your cortisol level to know if you're overtraining, undersleeping, or overwhelmed. You already know that. Trust what you're actually feeling over a number you were never meant to be watching in the first place.
